Do the LTE predictions apply to all cases, whether they are on the Escalation Request list or any status ?? ​

Yes, According to the current application behaviour a case will be predicted for LTE until the case is closed or it is in the active escalations list. Regardless of the case status, the cases that  have probability for escalations will be predicted by the ML as Likely to Escalate.

The cases that are in the escalation request list will also be predicted for the LTE.Hence unless the case is closed or as long as the case is not officially an "Active Escalation " the LTE will be predicted.
